Bridging Care Solutions provides both short- and long-term guidance through Support Coordination (Level 1 and Level 2) and Psychosocial Recovery Coaching.
Under the 0106 registration, Bridging Care Solutions supports participants to understand their NDIS plans and connect with appropriate services to meet their goals and support needs. Our supports are focused on building participants’ skills, confidence, and knowledge so they can manage significant life stages and transitions with greater independence.
We work alongside participants to coordinate multiple services and develop practical skills that support informed decision-making, reduce overwhelm, and promote choice and control. Our approach enables participants to live at home or in the community, remain engaged in daily life, and pursue their personal goals with confidence.
Supports include:
Support Coordination (Level 1 and Level 2)
Psychosocial Recovery Coaching
Assistance with understanding and managing NDIS support plans
Support during key life transitions (e.g. leaving school, moving out of home, starting or changing employment, transitioning between services)
Linking participants with appropriate community, mainstream, and NDIS-funded supports
Building skills to manage supports and make informed choices
Developing routines, planning skills, and decision-making capacity
Monitoring supports to ensure they align with participant goals
Assistance with problem-solving when supports change or challenges arise
