Bridging Care Solutions supports participants with day-to-day personal activities where assistance is required, including participants with high and complex support needs. Supports are delivered in accordance with individual support and care plans to ensure safe, consistent, and person-centred care.
By assisting with essential self-care tasks, we support participants to live safely, maintain dignity, and participate in daily life, both at home and in the community, in line with their NDIS goals and preferences.
Supports include:
Assistance with personal hygiene (showering, bathing, grooming, oral care)
Support with dressing and undressing
Assistance with toileting and continence-related tasks
Support with mobility and transfers
Assistance with eating and drinking (non-clinical)
Support with daily routines and self-care tasks
Assistance to participate in community activities where personal support is required
