Bridging Care Solutions offers dedicated one-on-one and small-group supports designed to assist participants to take part in community, social, recreational, and civic activities that are meaningful to them. Supports may include a support worker, mentor, or coach accompanying participants to a range of activities.
Our focus is on increasing community inclusion, strengthening social connections, and supporting active participation in everyday life, in line with each participant’s goals and individual support needs.
Supports include:
Support to attend community, social, and recreational activities
Assistance to participate in civic activities (e.g. volunteering, community events, interest groups)
One-on-one or small-group community participation supports
Support to build social skills and confidence in community settings
Assistance to develop and maintain relationships and social networks
Support to access community facilities and services
Encouragement and assistance to engage in hobbies, sports, and cultural activities
